Statement of Claim
Claimant says:
"HERE IS THE TIMELINE BELOW TO THE CHARGES IN DISPUTE:

03/16/2012 THE HOME DEPOT 3647 WAKEFOREST USA $243.17
03/18/2012 CVS PHARMACY #8959 YOUNGSVILLE USA $140.42
03/16/2012 CVS PHARMACY #1395 DURHAM USA $190.06
03/19/2012 FOOD LION 2686 YOUNGSVILLE USA $36.52

1. I will now allow myself to get pushed around by Achieve. I legitimately lost my card and called you the same day.

2. On that phone call I was asked if I wanted a new card to be sent to me and I did.

3. Get my statement 2 + weeks later with these (4) charges that are not mine.

4. Called Achieve and was told nothing had been done with the lost card - it wasn't closed at all still able to charge on it even after my call, then they told me my replacement card hadn't been sent yet. So all this time had gone by, my card was used it looks like 2 or 3 other times after I called you all.

5. They sent me documentation to fill out via e-mail - I filled all (4) forms out with all the information including but not limited to time, date and place of purchase based on the information I received from Achieve.

6. Called back 2 weeks later didn't get the fax - sent it again.

7. Called back again still did not get my transmittal with the required information. Faxed it a 3rd time, but this time got the actual name of someone who was looking out for it because of the lack of follow through there.

8. Then you send me an e-mail with a letter attached weeks later telling me you are not putting the money back into my account cause I didn't contact you. How many times does a customer have to contact you, for the job to get done. If the card would have been closed properly there would have only been (1) charge not (4)....I am not suffering because Achieve screwed up.

Straighten it out immediately, I have waited long enough. I am not going away till this gets done and I get my money back. This is the worst customer service I have ever heard. Usually your Master Card company sticks up for their customer, what the hell."
