Statement of Claim
Claimant says:
"On November 17, 2011 I was charge I was charge $70.00 co-pay in error. My dental insurance plan paid Cherry Hill Family Dental on 12-27-11 $70.00 also. They agreed that I was overcharged and would send me a check for the $70.00. When I call the manager Jon told me it is the Corporate Office that will send it to me. They will not tell me the name of the corporate Office or telephone number. Bill was paid with my Visa card and I asked them to credit the card. They would not do that either. Every time I called I got no satisfaction and it becomes more & More frustrating. As of today (1-23-2012) I have not received anything. The office never calls back. I am a senior Citizen with a limited income and want my money back. This has been going on for over ten weeks. Please help me. Jon sometimes tells me it is in the mail. I never get it. Why are they dangling me arounnd. Please, Please help. Their office number is 856-857-0033."
