Claimant says:
"My complainnt is with DERMACARE in Scottsdale,AZ
On March 3,2016, I paid them in advance for 6 sessions of MICRO-INJECTIONS for fat dissolving and skin tighening.
I only received 3 treatments.  Because I live almost 30 miles from the office in Scottsdale, the owner suggested that I come to his Mesa office because the distance was less than 30 miles. 
I received two treatments in Scottsdale and one in Mesa.  I made additional appointments to be treated in Mesa.  When i arrived and waited for 45 minutes, I finally called the owner Carl, who informed that the NP would not be there on that day.  But to make another appointment, which I did. While enroute to my appointment, Carl called to inform me that he to cancel again (mother was ill).
I called numerous times to reschedule but Carl never returned my calls.  
ON 3/3/16 I paid him $1258 for 3 sssions and on 3/16/16, I paid $1000 for 3 more sessions. 
According to my records Dermacare owes me a refund of $752.00"
