Statement of Claim
Claimant says:
"1. I had a catract surgery on my left t eye in 2013 at Johns Hopkins Hospital.

2. I received numerous bills both for physicans and hospital services.
3. I paid all bills except for $556.40.

4. On 5/23/13, Johns Hopkins agreed that I should pay by instaljment of $100.00 permonth on 15th of each month.
5. I did so and have both the Johns Hopkins sstatements and my cancelled checks to show that I fully met my obligations.
6. On 8/7/14 I received a phone call from an agent of Jhons Hopkins, a collector, who said he was acting on behalf of Jhons Hopkins (Nationwide Credit Corp.) He said I owed $356.40 to Johns Hopkins. My daughter was at the time visinting me from California. Shegot my debit card and paid the the amount in full to collector.
7. However, I was sure I had paid in full as agreed by Johns Hopkins. I began a search of my 2013 tax documents. I am blind in one eye. The blindness was done by Johns Hopkins physicans who have since disappeared and no one knows about their whereabouts. They had an experimental project and pushed a needle wrongly into my eye. My left eye is suffering from macular degeneration. To check documents is rather hard for me but I use a magnifying glass. I searched for 10 hours and came with all John Hopkins statements that showed i had fullfilled my obligation.
8. However, the collector demanded further prooof by supplying cancelled checks. Johns Hopkins agent was unwilling to accept Johns Hopkins statements as proof. My daughter went through 2 banks statements (Citi and SDFCU)on line and discovered the cancelled checks. But the collector demanded that they be faxed to him.
9. We went to Staples and faxed all Johns Hopkins statements and my cancelled checks. Johns Hopkins' agent, the collector, said JHU will pay us back.
10. On 9/22/14 Johns Hokins agent, Nationwide Credit Corp, a collector, deposited $200 in my account. When contacted, the collector said Johns Hopkins claims one or more of my checks had bounced. Neither Johns Hopkins nor Citibank or SDFCU's statements show a bounced check.
11. When we complained Johns Hopkins agent, Nationwide Credit Corp send me a bill for the cost of collectors! I had not hired a collector."
